We are preferred providers on many commercial medical plans. It is, however, each patient’s responsibility to contact their insurance company to make sure our providers are covered under an individual plan. This includes: Thunder Pain and Wellness, Dr. Kenneth Thunder, Dr. Amanda Melone, and Pearla McDonald.

Dr. Thunder and Pearla McDonald are Medicare providers; Dr. Melone is not. The practice does not take Medicaid.

Automobile Accidents, Worker’s Compensation, Personal Injury, and Disability

Thunder Pain and Wellness is committed to helping as many people as possible. Because we’re a small office with limited resources, we’re unable to manage the vast amounts of time and paperwork associated with automobile accidents, worker’s compensation, personal injury, or disability. If you currently have an open claim, we regret to inform you that unless we are considered preferred providers under your worker’s compensation plan, we’ll be unable to take you as a patient at this time. Please check back with us when your situation has changed.

Self-Pay Prices

  • Initial Visit: $400
  • Follow-Up Visit: $200
  • Injections (trigger point, nerve blocks, etc): $200, plus cost of follow-up visit
  • Monovisc: $1350 (plus cost of injection)
  • Synvisc-1: $1100 (plus cost of injection)
  • Xeomin (botulinum toxin): $450 per 50 units (plus cost of injection)
  • Acupuncture (first 15 minutes): $45
  • Acupuncture (each additional 15 minute block): $35
  • Transcranial Magnetic Stimulation (TMS): $5000 for 36 sessions (see TMS protocol)
  • TMS Mapping, pay-as-you-go: $400
  • TMS Subsequent Sessions, pay-as-you-go: $250
  • Ketamine Infusion for Pain: $700
  • Ketamine Infusion for Depression: $450
  • Ketamine injection: $30
  • SPRAVATO (esketamine) Observation: $300 (does not include cost of medication)
  • SPRAVATO medication: $375 per 28mg
  • Manual Therapy: $125
  • Vitamin B12 Injection: $40
